Information Regarding Your First Appointment
The initial consultation is always free. At this appointment I will discuss all fees with you and will always put the fee agreement in writing. There will be no hidden expenses.
If and when you decide to retain my services, I usually ask for a initial retainer of $200.00, so that we may order your credit report and provide you with a “Client Code” to complete the required pre-filing credit counseling course.

The new bankruptcy law requires that I review how much you have earned in the last six months. Please bring in all of your paycheck stubs for the last six months or, preferably, a printout from your employer’s payroll department. If you are in business for yourself, please bring in a profit and loss statement on a month-by-month basis for the past six months. If you are self-employed, your bank statements may help deduce your gross income.
I will need a complete list of your creditors to complete your paperwork. This list will include names, addresses, account numbers and balances, as well as any attorneys or collection agencies. Click here for our Creditor List form.
Additionally, here is some more items you may wish to bring with you:
- Income tax returns for the last four years;
- Lawsuits, including foreclosures and garnishments;
- Divorce paperwork, if any;
- Real Estate tax bill from the County Assessors Office;
- A deed or abstract from each piece of real estate you own, reflecting the Legal Description of the property (e.g.; Lot 42 of the Homewood Addition to the City of Moline);
- Statements reflecting the balance of IRA or other retirement accounts.
